Née à Shanghai en 1925, Li Lihua fut surnommée Xiao Mi (Petit Chaton) en raison de sa naissance prématurée. Ses parents étaient tous deux des acteurs de l’opéra de Pékin (Zhang Shao Chuan et Li Gui Fung) et bien qu’ils n’étaient pas très chauds à ce que leur fille suive leur traces elle partit à Pékin à l’âge de 12 ans pour apprendre cet art.
A 15 ans elle retourne à Shanghai et fait connaissance avec Yan Chungtang, le dirigeant de la "Yihua Film Company", qui la poussa à entrer dans l’industrie cinématographique. Son premier film fut The Three Smiles réalisé en 1940 et en fit une star immédiate. En 1941 Shanghai est occupé par l’armée Japonaise et les compagnies cinématographiques réunies sous une seule entité, la Huaying. Li Lihua y travailla et à la libération fut accusée de traîtrise à cause de cela.
En 1949 elle est invitée à Hong Kong par la "Yung Hwa Motion Picture". Avec le temps elle accrut sa popularité et se mit à travailler pour d’autres compagnies, telles les fameuses Shaw Brothers et Cathay. Elle fonda sa propre compagnie avec son époux Yan Jun qui produisit quelques films avant de se rallier à la Shaw Brothers. En 1964 et 1969 elle gagna le Golden Horse de la meilleure actrice pour Between Tears and Laughter et The Yangtse Incident. Elle fit même en 1958 un film aux USA appelé China Doll aux cotés de Victor Mature.
Au début des années 70 elle partit avec son mari à New York où elle vécut environ 10 ans, ne faisant que quelques caméos dans une poignée de films hongkongais, jusqu'à ce que son mari décède d’une crise cardiaque. Elle se remaria et en 1994 enregistra même un nouvel album à Singapour. Elle coule une retraite paisible là-bas apparaissant de temps en temps dans des œuvres de charités et se consacrant à la calligraphie et à sa première passion : l’opéra.
